Client Forms

All clients must fill out the Massage Intake & Liability form prior to their first appointment. This form only needs to be completed one time for each client.

This form must be completed by a parent or guardian of any minor-aged client prior to their first appointment. This form only needs to be completed one time for each client (as applicable). 

Any clients who are pregnant must complete this form prior to their first prenatal appointment. This form only needs to be completed one time for each client (as applicable).
